Comprehensive Process

1. A family member or patient will call or e-mail us to discuss our services, or a hospital Case Manager will call us on behalf of the patient prior to discharge to ensure safe transition to home after a hospital stay.

2. A HomeWell representative and a Registered Nurse (RN) will come to your home or hospital to conduct a thorough needs assessment.  There is no charge for this service and no further obligation on your part.

3. HomeWell’s Case Manager and the RN will develop a Care Plan and present that information to the patient and/or family members.  The Care Plan will include all details of care and a recommended schedule.

4. If there is agreement to proceed, a simple 1-page Services Agreement is signed and home care will begin as planned.

5. On the first day of service, the HomeWell Case Manager and Caregiver will meet at the house to review the Care Plan, orientation of the residence and introduction to the patient and other family.

6. The Caregiver will conduct a daily satisfaction survey to ensure quality care delivery.

7. Supervisory visits by management are conducted every 60-90 days or more frequently if needed to update the plan of care.

 

Industry Background Information

A study by the AARP in 2005 indicated that 89% of people who are 50 years old or older want to remain at home as long as possible.  Geriatricians increasingly believe that older people who stay in their own homes do better emotionally and physically than those who live in nursing homes or other senior living facilities.  (Wall Street Journal, May 18, 2006)

 

In addition to the aging population, growth in the home care industry is expected to come from the patient preference for health care delivered at home, changing family structures, medical and technological advances, an expanding marketplace for specialized home care services, increasing physician acceptance of home care, and the cost advantage of home care over institutional care.

 

According to the most recent estimates prepared by the National Association for Home Care and Hospice, costs for a hospital day and a skilled nursing facility day are estimated at more than $3,800 and $490, respectively, while the average home care visit is just over $100.

 

The U.S. Center for Medicare & Medicaid Services (CMS) anticipates national home health expenditures, including durable medical equipment, will rise from more than $84 billion in 2007 to over $148 billion by 2016.  The U.S. Departments of Labor and Health and Human Services have estimated that by 2050, the home health industry will require 13 times the half million workers it employed in 2000.
